Tribal Health Improvement Program

THIP is a collaborative initiative between the Tribal, State, and Federal governments aimed at addressing health disparities among American Indian Medicaid recipients residing on reservations. The program selects clients from the State Medicaid program as the top 25% members with chronic illnesses or at risk of developing serious health conditions through intensive care coordination. 

Purpose

 

Tribal Health Improvement Program (THIP) serves as a comprehensive health promotion and disease prevention program. Its objective is to empower individuals to make informed healthcare decisions, coordinate their care effectively, and implement wellness strategies.  

Who we serve
 

Tribal Health Improvement Program (THIP) is designed to serve individuals residing on reservations who are eligible for Medicaid and have chronic illnesses or are at risk of developing serious health conditions.

In our Tribal Health Improvement Program Task Order states the agreement was Effective November 1, 2020 

How to join
 

The state selects the potential clients who have chronic illnesses and are given at least 10 Medicaid members at the beginning of the month for Care Coordinators or Outreach Coordinators to contact. To help improve clients’ health we have Wellness Coordinators and our Cultural Coordinator to help set goals to reach pertaining to mental, physical, emotional and spiritual.
